Requests for disclosure, etc., should be directed to the Personal Information Inquiry Desk mentioned above. If you preferdisclosure through electronic procedures, please indicate this when making the request. In principle, we will process yourrequest accordingly.

Procedures for Requests for Disclosure, etc.
a. Upon receiving your request, we will mail you the designated request form, "Request for Disclosure of Retained Personal Information."
b. Please complete the request form and mail it to the Personal Information Inquiry Desk along with documents verifying an agent’s authority (if applicable) and a postal money order for the handling fee (applicable only for requests for notification of the purpose of use or disclosure).
c. Upon receiving the request form, we will verify your identity by requesting two pieces of information that match our records (e.g., phone number and date of birth).
d. Responses will generally be provided in writing (sent by postal mail) directly to the requester. -
Requests Made by an Authorized Representative
If a request is made by an authorized representative, documents verifying both the representative’s authority and identity must be enclosed.
The information on the applicant’s registered domicile should be limited to the prefecture level, with any further details redacted (e.g., blacked out).
Documents should not contain an individual number (My Number). If they do, the number should be completely redacted.

4. Fee
For requests for notification of the purpose of use or disclosure, a fee of
1,000 yen will be charged for each request.Please enclose a postal money order

We take precautions against the theft or loss of devices, electronic media, and documents containing personal data, and when transporting such items within the company or outside, we ensure that personal data is not easily identifiable. -

Access control is implemented to limit the scope of responsible personnel and personal information databases they can handle.
We have introduced systems to protect information systems handling personal data from unauthorized external access or malicious software.
